DESCRIPTION:
Amoxicillin is a broad-spectrum semisynthetic antibiotic. Amoxicillin binds to penicillin-binding protein 1 A (PBP-1 A) located inside the bacterial cell wall. Penicillins acylate the penicillin-sensitive transpeptidase C-terminal domain by opening the lactam ring. This activation of the enzyme prevents the formation of a cross-link of two linear peptidoglycan strands, inhibiting the third and last stage of bacterial cell wall synthesis. Cell lysis is then mediated by bacterial cell wall autolytic enzyme such as autolysins.
INDICATIONS:
– Respiratory tract – pneumonia, bronchitis
– ENT – otitis media
– Urinary tract – cystitis, pyelonephritis
– Biliary and intra-abdominal infections
– Gynaecological infections
– Gonorrhoea
– Septicaemia
– Bacterial endocarditis
– Skin and soft tissue infections
– Meningitis (seek expert advice)
– Enteric fevers (typhoid and paratyphoid fevers- seek expert advice)
– Dental abscess (as an adjunct to surgical management)
CONTRAINDICATIONS:
Moxytro capsules 250 mg/ 500 mg is contraindicated in patients with hypersensitivity to penicillins. Attention should also be paid to possible cross-reactivity with other beta-lactam antibiotics e.g. cephalosporins. It should not be given to patients with infectious mononucleosis (glandular fever) since they are especially susceptible to amoxicillin-induced skin rashes.
SIDE EFFECTS:
Allregic reactions may occur. Skin rashes (pruritic skin rash, and erythrematous skin reaction or urticaria) are the most common side-effects. Fever, eosinophilia, joint pains, angioneurotic oedema, erythema multiforme and anaphylaxis may occur.
